Please contact your congressman for your district.  They will refer you to their website, or They will sent you an E-Mail with a letter which has an electronic form which you complete and send it back electronically via your E-mail.

You may want to state: If you have not made any changes to your information in the past year.  If you have made changes state the changes.  State if you were born in the United States.
