Complete a "Five Wishes" Advance Directive

Complete a “Five Wishes” advance directive with your loved one, and don’t worry about a separate “Living Will”, because those end of life questions are included in the Five Wishes.  Once it is completed, you only need 2 witnesses who are not related to the care recipient. Then photocopy and make sure every interested member of the family gets a copy, as well as the main health providers involved in their care.
