The wording would be something like this - “If I cannot be restored to my current self, take no action to save me”   OR

If I cannot be a fully functioning person, meaning that I can take care of my activities of daily living, that I can make decisions for myself, then I elect not to have any life-saving procedures taken to save me and I do not wish to have any artificial food or artificial manner of feeding me.  

This could be as detailed as one would want to be - 

I don’t have to have any cures that I do not want either.  

Problem is, without an enforceable legal document, a person or entity that could be in charge of your care or just where you might be  (person, SNF, hospital, Alzheimer’s facility,) may opt to not watch one starve and thus would disregard the wishes that have been put forth.  States normally have a hard time enforcing certain wishes.  

My God will know when the time is right for me - I just don’t want some other human making any decisions that would delay My God’s will (or mine).

My God understands that I do not want to live if I am not a functioning human being and we have an understanding.  Life is defined and it is more than just taking a breath in and then pushing it out. Pretty much all or nothing.

So the problem is My God’s will and my wishes vs all those other’s out there.  So I would opt to document that when I get to the point (defined specifically) then I would want some M.A.I.D. of whatever type is necessary under my own accord if possible, but if not, with some assistance, to hurry up the process because that would be the most compassionate thing to do at the time.

Problem is, at that point, it isn’t the patient making the decision - it might be their family.  And unless there is a (personal) legal, binding and enforceable document that overrides their decision - then it is the family’s or a member of it, decision.  They might want you to stick around in whatever shape because they cannot stand to be without you (you at this point - is what?)

A question - If you have a fatal disease to which there is no cure or you have tried all the cures and death is imminent - would you opt for hospice care?  

What does Hospice do - they give you palliative care at the end of life.  That includes keeping you comfortable and pain free and without anxiety.  Now when the time comes that perhaps you have to be given so much medication to keep you from waking up because if you do you would feel as though you are suffocating - they increase the dose incrementally to keep you in that state - free from pain, anxiety and comfortable - until nature takes it course - but is it really nature?  Is it God?  Is it just too much medication? 

Anyway you look at it - it is done and with compassion - 

Personally, I think there are so many different scenarios that could happen that nobody can plan for every little thing - but I am gonna do my best to make it as good as it can be for me.

Otherwise, I might just be at home - like my grandmother who just went to sleep and never woke back up -
