There's a window for applying for Medicare, if I apply can I put it on hold?

I understand that there's a window of opportunity around my 65th birthday to apply for Medicare.

If I apply and get approved, does it automatically begin, or can I suspend it from beginning, until when I choose to begin it's coverage? Or once approved it becomes my primary insurance?
