United Healthcare medicare advantage REWARDS FOR 4 SCREENINGS

rec'd brochure with forms for wellness exam, mammogram, colon cancer, glaucoma screening to be completed by member and doctor, along with results of  various lab tests.  Completed forms  to be returned to UHC in order to qualify for Visa debit card for $40 total for all 4 screenings.  Does anyone know why UHC is doing this?

This is what I found today.  When you are in the United Healthcare  website, I reedemed an order for an Applebees gift card. Look for Renew Rewards. Hope it helps you. It took me a month to find.

CALL THE UHC PHONE NUMBER ON YOUR MEDICARE ADVANTAGE ID CARD TO SEE IF THIS PROGRAM IS STILL IN EXISTANCE AT ALL OR IF IT IS IN EXISTANCE UNDER YOUR AARP/UHC  MA HEALTH PLAN.

THE LAST INFO THAT I CAN FIND ON THIS PARTICULAR PROGRAM WAS BACK IN 2015.
