Keeping an eye out

I have an older family member who needs to be constantly monitored due to a medial illness.

I am looking for some sort of monitoring system that has:

1. A well-made, reliable & easy to move (if need be) camera w/good resolution & sound pick-up.

     (corded/wireless is not a big concern.  If it can be hung on a wall, that's a plus).

2. Able to connect to a screen/tablet/phone by Bluetooth.

     (if a screen/tablet are included, that would be great).
